Why Neck Pain Spreads to the Shoulder: Causes, Nerve Irritation, and Muscle Overload

However, when the pain spreads from the neck into the shoulder, there are usually two main causes: nerve irritation from the neck or *muscle overload in the neck and shoulder.*

Causes of Neck Pain That Goes Into the Shoulder

1. Nerve Irritation from the Neck

Irritation often involves the nerves that originate from the neck. These nerves travel from the neck into the shoulder, arm, elbow, wrist, and fingers.

When a nerve becomes compressed, irritated, or placed under tension, pain may start in the neck and extend into the shoulder. One common reason for this is disc-related pressure in the cervical spine, where a disc reduces the space around the nerve and causes irritation.

2. Muscle Overload in the Neck and Shoulder

The second cause is muscle overload. Muscles of the neck are closely connected to the shoulder and arm. Some muscles begin in the neck and attach to the shoulder, while others continue from the shoulder down into the arm.

When these muscles become tight, fatigued, or overloaded, they can restrict neck movement and create pain that travels toward the shoulder. This commonly happens due to poor posture, prolonged computer or phone use, repetitive strain, or muscle tension.

This tightness can limit movement and make it painful to turn the head, bend the neck, or move the shoulder comfortably.

The neck and shoulder function as one biomechanical unit.

Because the nerves and muscles of the neck extend directly into the shoulder and arm, a problem in the neck can easily cause symptoms in the shoulder.

Both the nerves and muscles are interconnected, which is why neck pain can travel into the shoulder and sometimes even into the arm.
